About Our Clinical Services
The Fein Memory and Aging Center specializes in the diagnostic evaluation and treatment recommendations for neurodegenerative diseases, including:
- Common conditions: Alzheimer’s disease, vascular dementia, and dementia with Lewy bodies
- Less common disorders: Frontotemporal dementia, Huntington’s disease, and Creutzfeldt-Jakob disease
- Specialty care: Cognitive complaints associated with movement disorders or genetic conditions
Our multidisciplinary team is dedicated to providing comprehensive evaluations and tailored care plans for patients with cognitive and behavioral concerns.
Referral Process
To submit a referral, please complete the following:
- UCSF Edward and Pearl Fein Memory and Aging Center Clinic Referral Form
- UCSF Edward and Pearl Fein Memory and Aging Center Clinic Referral Checklist
In addition, please include the following materials to facilitate the review process:
- Recent progress notes outlining the patient’s condition and concerns
- Relevant lab results
- Available brain imaging (on CD if possible)
- Insurance information and authorization
Once we receive the referral, our team will carefully review it to determine the most appropriate evaluation. This review process typically takes a few weeks. Due to high demand, there may be a delay in scheduling. We appreciate your patience as we work to arrange the best appointment for your patient.
