The Care Ecosystem is a supportive care program for families living with dementia that improves the quality of life for patients and their caregivers while reducing emergency-related care. You can learn more about it here. We continue to refine this care model and study its effectiveness in diverse health settings and with a diverse patient population. Our team is also designing new care models to address gaps in care for people with dementia, including the Brain Health Consultation, which is designed to support families around the time of diagnosis, in partnership with primary care.
The Care Ecosystem is based at the Memory and Aging Center. Our Center is dedicated to providing the highest quality of care for individuals with cognitive problems, conducting research on causes and cures for degenerative brain diseases, and educating health professionals, patients and their families. Rich didactic and other training opportunities are available to fellows and can be tailored based on their career goals and interests. Dr. Kate Possin will be the primary mentor, and we will select additional mentors based on the fellow’s training goals.
