Thank you for considering a referral to the UCSF Memory and Aging Center. We appreciate the opportunity to provide consultation services to you and your patients.
The clinical services at the Memory and Aging Center are focused on providing diagnostic evaluation and treatment recommendations for neurodegenerative diseases. This includes diagnoses such as Alzheimer’s disease, vascular dementia, and dementia with Lewy bodies, as well as less common disorders such as frontotemporal dementia, Huntington’s disease, and Creutzfeldt-Jakob disease. We also have a specialty clinic for patients with cognitive complaints that may be associated with movement disorders or genetic conditions.
If you have questions about the process or would like to speak with someone about a referral, please call 415.353.2057. Once received, all referrals are carefully reviewed by our team to determine the most appropriate evaluation. Please complete the following:

Please also provide the following information to facilitate review:
- Recent progress notes describing the problem
- Any pertinent lab work
- Any available brain imaging (actual film on CD)
- Insurance information and authorization
This thorough process usually takes a couple of weeks, and due to a large volume, there may be a delay until your patient is contacted for scheduling. We would appreciate your patience as we arrange the best appointment for your patient. We understand there are varying degrees of urgency for patient consultations. If a patient requires more urgent attention, please call the center to speak with one of our clinicians. We will do our best to accommodate urgent requests.
