Preparation for Your Appointment

To schedule an evaluation at the Fein Memory and Aging Center, follow these steps:

  1. Referral: Have your referring physician send a referral letter, insurance information, and authorization to the clinic. The letter should explain the reason for referral and summarize your condition.
  2. Review: Our clinic staff will review the referral and medical records to confirm if this service is appropriate for you.
  3. Scheduling: If accepted, a patient care coordinator will contact you to schedule an appointment. Please note that new patient appointments are typically booked several months in advance.

Evaluation

At the appointment, each patient undergoes a comprehensive evaluation, which typically lasts 2–4 hours and includes:

  • Neurological assessment
  • Cognitive testing
  • Detailed medical history, including input from someone who knows the patient well

Our multidisciplinary approach helps identify the cause(s) of the patient’s symptoms and provides personalized treatment recommendations. Feedback may include a preliminary diagnosis or deferral of diagnosis until additional information is gathered. A detailed written report with results and recommendations will be sent to the referring physician after clinical consensus is achieved.

Interpretation Services

Language should never be a barrier to receiving care. UCSF Health offers free interpreter services in multiple languages, including Mandarin, Cantonese, Vietnamese, Spanish, Russian, and others. The Fein Memory and Aging Center staff also reflects diverse cultural backgrounds to ensure inclusive care.
